In just six simple steps, you can implement a robust wellness visit program and begin delivering better patient care and generating significant revenue.


Step 1 – Implementation:

Our solution is cloud-based, so you do not need to buy any hardware or install software. As a result, our solution can be implemented quickly with minimal distraction to your practice.


Step 2 – Training:

Our clinical staff trains you to use our software and efficiently deliver the wellness visit. Training can be done quickly since our software is very intuitive and is based on established medical protocols and Medicare guidelines. We also provide comprehensive training materials that you can reference after the initial training.


Step 3 – Marketing:

We work with you to implement an effective marketing strategy to target existing and new Medicare patients. You will be pleasantly surprised how quickly your calendar will start filling up with scheduled wellness visits.


Step 4 – The Wellness Visit:

Using our innovative software, you deliver the wellness visit in just 20 minutes. Your patient receives a printed personalized prevention plan (PPP) and a referral for any indicated preventive screenings or additional tests to detect early chronic disease. At the conclusion of each wellness visit, our system enables you to export all the information relating to the wellness visit in to virtually any EHR/EMR system in either continuity of care record (CCR) or PDF format. Custom integration options are also available.


Step 5 – Payment:

To help ensure that you receive the appropriate reimbursement for the wellness visit and any additional preventive services or other tests that are identified during the wellness visit to detect early chronic disease, relevant procedural codes (HCPCS and CPT codes) and codes that identify the particular diagnosis (ICD-9 codes) are provided.


Step 6 – Post-Wellness Visit:

You will have access to various program evaluation tools to objectively analyze outcome measures, patient health improvement and program performance. Your patients have access to our wellness portal that contains helpful resources such as instructional exercise videos & nutritional information; information related to illnesses affecting seniors; government, non-profit and community based resources; and other relevant tools, resources & information. You and your patients are prompted to schedule subsequent visits 12 months after the initial wellness visit.
